(REQUIRED) 2008-09 Roommate Assignment Form (REQUIRED) This form is important because it tells us what type of housing and/or roommates you are interested in. To finalize your contract, you must submit an application, along with $150.00 damage deposit, minimum applicable monthly payment (Refer to Fee Schedule), a $25.00 non-refundable application fee, and (optional) $20.00 activity fee to the: Office of Housing and Residential Life 6000 University Parkway San Bernardino, CA 92407-2307 Make checks or money orders payable to: CSUSB - Housing and Residential Life. Please do not send cash. Credit cards cannot be used for the 1st payment. Confirmation letters will be sent in mid August.
